Welcome to Pure Health Center! If you are a new client please complete the online new client questionnaire by clicking the appropriate button to the right. Please make sure you have also booked an appointment online, contacted your therapist of choice directly or reached out to info@purehealthcenter.com to set up an appointment.
West Loop clients please make sure you receive call box code from your therapist or search your therapist's name on call box. (Please make sure you do not follow someone else in as you will not have access to the 3rd floor from the elevator.) When you come up to the 3rd floor walk past the restroom and Suite 302 will be on your left. Please have a seat in our lobby and help yourself to a refreshment!
Roscoe Village and Arlington Heights clients please make sure you have received a private door access code from your therapist as main door will be locked for security when you arrive. Please find a seat in our lobby and help yourself to a refreshment! Restrooms are located next to the lobby in both locations.
All of our licensed psychotherapists are in network with Blue Cross Blue Shield PPO and BlueChoice plans. Please verify your insurance coverage prior to your appointment. If you would like us to look up for you please fill out the information in the online questionnaire. However, please note you will be responsible for any portion of the session not covered by insurance for any reason. If you have an out of network insurance plan and will be paying out of pocket, we are happy to provide receipts that you may send to your insurance company for any potential out of network reimbursement. Self-pay rate for psychotherapy is $175/session.
Our graduate interns do not accept insurance, but instead offer services on a sliding scale based on income between $30 and $50.
